Student athletes and parent/guardians must complete all paperwork and submit payment (credit/debit card) using the online system.

Payments can be brought to the Activities Office; however, paperwork still must be completed online.
New physical information must be handed in first and entered into the system before continuing the registration portion. Physicals can be emailed to natalie.crosby@isd623.org.
Transfer students must submit a current physical.
Students with outstanding fines/fees must pay and be cleared before students are able to register.
You do not need to stop in the Activity Office to pick up a clearance form. Your confirmation email signifies a successful registration.

All students must have a sports qualifying physical exam on file in the activity office at RAMS (for grades 7-8) or at RAHS (grades 9-12) prior to playing interscholastic athletics. The physical form must state that the student is cleared by their healthcare provider to play sports. Physicals are valid for three years. If a physical expires during the upcoming sport season, a new physical is required before a student will be allowed to register.
Any student-athlete whose physical exam date has not been entered into the registration program cannot proceed through the registration process. Once the date of sports clearance has been recorded in the RAMS or RAHS athletic office, the original form is placed in the student’s health record. Contact your student’s school health office if you need a copy.
MN State High School League Sports Qualifying Physical Examination Medical Eligibility form is available online, in the activity offices, or at your school health office. Your healthcare provider may have their own sport qualifying form, and that is acceptable if the provider specifies that the student is cleared for their sport, contains the date of the exam, and the form is signed by that healthcare provider.
Each spring and early summer, there are clinics in our area that offer free or reduced cost appointments for sports physicals. Watch for district communications when those opportunities become available.

If a 7th- or 8th-grade student is on a 7-12 team such as cross-country running, swimming, dance, nordic ski racing, alpine ski, or gymnastics, the $200 fee applies.
If a student moves out of the district or is cut from the team, 100% of the fee will be refundable.
A refund request made for these reasons must take place prior to the 3rd official contest of the season .

Due to the Data privacy Act, parents must inform the Athletic Office if they qualify. Students or families with other financial difficulties should contact the School Activities Director to discuss reduced or waived fees.
